SRI DEVARAJ URS ACADEMY OF HIGHER EDUCATION AND RESEARCH invites application for Fellowship program in Laboratory Medicine and Laboratory Management.

Eligibility : M.D. / DNB in the subjects of Pathology, Microbiology, Biochemistry, Transfusion Medicine.

Or

DCP with one year experience.

Duration of the Programme : One year

Fee structure : Course fee Rs.70,000/-.

Issue of application forms : 10.10.2018

Last date for receipt of filled-in application forms : 15.11.2018

Date of Entrance examination : 19.11.2018

Date of commencement of course: 03.12.2018

Interested candidates are advised to submit pertinent information in the application form available on the website www.sduu.ac.in of The Academy on or before 15.11.2018 along with an application fee of Rs.1000/- drawn in favour of SDUAHER payable at Kolar. Applications should accompany attested copies of relevant documents in support of the eligibility criteria and should be sent to the Registrar, Sri Devaraj Urs Academy of Higher Education and Research, Kolar.

List of documents to be enclosed along with the application form:

1. Under Graduate / Post Graduate Degree marks cards.
2. Under Graduate / Post Graduate Degree Certificates.
3. No Objection Certificate from the Head of the department and the institute where he/she is employed.
4. Passport size Photograph (5 Nos.) of the candidate.
5. Application fee of Rs.1,000/- in the form of DD drawn in favor of Sri Devaraj Urs Academy of Higher Education and Research, payable at Kolar.

Attach only Photo copies of Marks card, Degree certificates or of any other documents. Produce the originals at the time of interview.
